Creative Community Services, Inc. (CCS) is a therapeutic foster care agency serving children, teens, and adults with developmental disabilities. For over 40 years, we have served Georgia’s most vulnerable populations through our programs and services.
The Family Consultant is a key member of the clinical team, responsible for coordinating day-to-day activities for individuals with developmental disabilities (DD). This role involves managing assigned regions, overseeing the implementation of Individual Service Plans (ISPs), and ensuring a safe, growth-promoting environment for individuals. The Family Consultant reports to the Family Consultant Supervisor.
Key Responsibilities:
- Manage contracts and waiver services for assigned regions.
- Ensure safe and supportive living environments for individuals.
- Complete monthly supervision checklists for residential services.
- Coordinate staff hiring, training, and monthly family/team communications.
- Attend Intake and ISP meetings, leading intervention and service implementation.
- Ensure accurate Medicaid billing and financial documentation.
- Participate in on-call rotations (Mon 8:30 AM – Fri 5:00 PM) and emergency staffing as needed.
- Provide direct support to individuals, with at least one monthly contact in the community or home.
- Supervise direct care staff and complete necessary case documentation (SOAP notes, case logs, etc.).
- Ensure individuals attend CCS social events and coordinate transportation as needed.
- Maintain and update individual charts, ensuring accurate documentation of services and progress.
- Adhere to CCS policies, including dress code and timely filing of documentation.
- Participate in weekly supervision and attend monthly unit and staff meetings.
- Provide additional training to care assistants (CAs) as needed.
- Respond to incidents and report to the Individual Advocate/FC Supervisor within 24 hours.
Desired Qualifications:
- Bachelor’s degree in human services or related field, or experience working with individuals with developmental disabilities.
- Ability to analyze and respond to challenging situations, often under stress.
- Knowledge of DD systems, DBHDD regulations, and Office of Regulatory Services guidelines.
- Strong organizational skills and the ability to manage multiple tasks efficiently.
Physical Requirements:
- Ability to perform essential job functions, including sitting, standing, walking, and communicating.
- Responsible for providing transportation and ensuring individual safety, especially in inclement weather.
